Outreach

It is a specialist service designed to provide you with support in your home and in the community if you have learning disabilities. You are supported to live as independently as possible with flexible support dependent on your individual needs and ranging from 1 hour to round the clock 24 hrs a day.

We provide outreach for people with learning disabilities and engage them in community based activities.

At Care Matters Too, we believe that every person deserves the chance to achieve their fullest potential and we encourage positive risk taking to help you maximise your independence and control of your life; to build resilience; and where needed, to build on your recovery and rehabilitation. Our team will work with you and any other professionals involved in your care to agree on goals and outcomes that we will support you to work towards.

We understand that finding the right support for people who need specialist care can be difficult. So, we provide experienced specialist carers to make sure that you feel supported every step of the way.

We provide personalised support with:

Developing and maintaining life skills, such as food shopping, cooking and household chores.

Help with finances and budgeting, and maintain a tenancy.

Healthy lifestyles such as food and nutrition planning, sporting activities.

Voluntary or work placements.

Travel training.

Social life and leisure activities such as attending clubs, organising trips and holidays.

College activities and college work.

Our support team can support people with particular aspects of day-to-day life such as managing money, staying healthy, and learning cooking skills – whatever is needed to live fully.
